• ராஷ்ட்ரிய பால் ஸ்வஸ்த்யா காரியக்ரம் (RBSK) என்பது, பிறந்தது முதல் 18 வயது வரையிலான குழந்தைகளை, 4 ‘D’களை உள்ளடக்கும் வகையில், முன்கூட்டியே அடையாளம் கண்டு, முன்கூட்டியே தலையிடுவதை நோக்கமாகக் கொண்ட ஒரு புதிய முயற்சியாகும். பிறப்பில் குறைபாடுகள், குறைபாடுகள், நோய்கள், இயலாமை உள்ளிட்ட வளர்ச்சி தாமதங்கள்.
• 0 - 6 வயதுக்குட்பட்டவர்கள் குறிப்பாக மாவட்ட ஆரம்ப தலையீட்டு மையம் (DEIC) மட்டத்தில் நிர்வகிக்கப்படுவார்கள், அதே நேரத்தில் 6 -18 வயதுக்குட்பட்டவர்களுக்கு, தற்போதுள்ள பொது சுகாதார வசதிகள் மூலம் நிலைமைகளை நிர்வகித்தல் என்பது குறிப்பிடத்தக்கது. இரு வயதினருக்கும் DEIC பரிந்துரை இணைப்புகளாக செயல்படும்.
• தற்போதுள்ள மருத்துவ அலுவலர்கள், பணியாளர் செவிலியர்கள் மற்றும் ANMகள் மூலம் அனைத்து டெலிவரி புள்ளிகளிலும் முதல் நிலை திரையிடல் செய்யப்பட உள்ளது. 
• 48 மணி நேரம் முதல் 6 வாரங்கள் வரை பிறந்த குழந்தைகளின் ஸ்கிரீனிங் HBNC தொகுப்பின் ஒரு பகுதியாக ஆஷாவால் வீட்டிலேயே செய்யப்படும்.
• 6 வாரங்கள் முதல் 6 ஆண்டுகள் வரை அங்கன்வாடி மையங்களிலும், 6 முதல் 18 வயது வரையிலான குழந்தைகள் பள்ளியிலும் பிரத்யேக மொபைல் பிளாக் அளவிலான குழுக்களால் அவுட்ரீச் ஸ்கிரீனிங் செய்யப்படும்.
• இந்த அடையாளப் புள்ளிகளில் ஏதேனும் இருந்து குழந்தை பரிசோதிக்கப்பட்டு பரிந்துரைக்கப்பட்டவுடன், குடும்பத்திற்குத் தேவையான சிகிச்சை/தலையீடு பூஜ்ஜிய செலவில் வழங்கப்படுவது உறுதி செய்யப்படும்.

வயது

• அரசு மற்றும் அரசு உதவிபெறும் பள்ளிகளில் ஒன்றிலிருந்து பன்னிரெண்டாம் வகுப்புகளில் சேரும் குழந்தைகளுக்கு கூடுதலாக கிராமப்புறங்கள் மற்றும் நகர்ப்புற குடிசைப் பகுதிகளில் உள்ள 0 - 6 வயதுடைய குழந்தைகளை உள்ளடக்கும் வகையில் இந்தச் சேவைகள் வழங்கப்படுகின்றன. 
• இந்த சேவைகள் படிப்படியாக சுமார் 27 கோடி குழந்தைகளுக்கு சென்றடையும் என்று எதிர்பார்க்கப்படுகிறது. வயதுக் குழு மற்றும் மதிப்பிடப்பட்ட பயனாளிகளின் பரந்த வகை அட்டவணையில் கீழே காட்டப்பட்டுள்ளது. வெவ்வேறு கருவிகள் பயன்படுத்தப்படும் மற்றும் வெவ்வேறு நிபந்தனைகளுக்கு முன்னுரிமை அளிக்கப்படலாம் என்ற உண்மையின் காரணமாக குழந்தைகள் மூன்று வகைகளாகப் பிரிக்கப்பட்டுள்ளனர்.

சமூகம் மற்றும் வசதி அளவில் திரையிடலுக்கான வழிமுறைகள்

• RBSK இன் கீழ் குழந்தைத் திரையிடல் சமூக நிலை மற்றும் வசதி நிலை என இரண்டு நிலைகளில் உள்ளது. PHCகள் / CHCகள் / DH போன்ற பொது சுகாதார வசதிகளில் வசதி அடிப்படையிலான புதிதாகப் பிறந்த ஸ்கிரீனிங், தற்போதுள்ள மருத்துவ அலுவலர்கள், பணியாளர் செவிலியர்கள் மற்றும் ANMகள் போன்ற சுகாதார பணியாளர்களால் செய்யப்பட வேண்டும், சமூக அளவிலான திரையிடல் அங்கன்வாடி மையங்கள் மற்றும் அரசு மற்றும் அரசு மற்றும் மொபைல் சுகாதார குழுக்களால் நடத்தப்படும். அரசு உதவி பெறும் பள்ளிகள்.

அங்கன்வாடி மையத்தில் திரையிடல்

• 6 வயதுக்குட்பட்ட அனைத்து முன்பள்ளிக் குழந்தைகளும், ஆண்டுக்கு இரண்டு முறையாவது அங்கன்வாடி மையத்தில் குறைபாடுகள், நோய்கள், குறைபாடுகள் உள்ளிட்ட வளர்ச்சி தாமதங்கள் குறித்து மொபைல் பிளாக் சுகாதாரக் குழுக்களால் பரிசோதிக்கப்படும். 0-6 வருடங்களுக்கான திரையிடலுக்கான கருவியானது, குறிப்பாக வளர்ச்சி தாமதங்களுக்குப் பட, வேலை உதவிகளால் ஆதரிக்கப்படுகிறது. 
• வளர்ச்சி தாமதங்களுக்கு, குழந்தைகள் குறிப்பிட்ட வயதுக் கருவிகளைப் பயன்படுத்திப் பரிசோதிக்கப்படுவார்கள், மேலும் சந்தேகத்திற்குரியவர்கள் DEIC க்கு மேலதிக நிர்வாகத்திற்கு பரிந்துரைக்கப்படுவார்கள்.

பள்ளிகளில் திரையிடல் - அரசு மற்றும் அரசு உதவி பெறும்

• 6 முதல் 18 வயது வரை உள்ள பள்ளிக் குழந்தைகள், வருடத்திற்கு ஒரு முறையாவது உள்ளூர் பள்ளிகளில் குறைபாடுகள், நோய்கள், குறைபாடுகள், இளமைப் பருவத்தினரின் உடல்நலம் உள்ளிட்ட வளர்ச்சி தாமதங்கள் குறித்து மொபைல் ஹெல்த் குழுக்கள் மூலம் பரிசோதிக்கப்படும். 
• கேள்வித்தாள் (உள்ளூர் அல்லது பிராந்திய மொழியில் மொழிபெயர்க்கப்பட்டது) மற்றும் மருத்துவ பரிசோதனை ஆகியவையும் பயன்படுத்தப்படுகின்றன.

மொபைல் சுகாதார குழுவின் அமைப்பு

• மொபைல் ஹெல்த் டீம் நான்கு உறுப்பினர்களைக் கொண்டிருக்கும்- இரண்டு டாக்டர்கள் (ஆயுஷ்) ஒரு ஆண் மற்றும் ஒரு பெண், குறைந்தபட்சம் அங்கீகரிக்கப்பட்ட நிறுவனத்தில் இளங்கலை பட்டம் பெற்றவர், ஒரு ஏஎன்எம்/ஸ்டாஃப் நர்ஸ் மற்றும் டேட்டா மேனேஜ்மென்ட் கணினியில் தேர்ச்சி பெற்ற ஒரு மருந்தாளுனர்.

ENGLISH

• Rashtriya Bal Swasthya Karyakram (RBSK) is a new initiative aiming at early identification  and  early  intervention  for  children  from  birth  to  18  years to cover 4 ‘D’s viz. Defects at  birth,  Deficiencies,  Diseases,  Development  delays  including  disability.
• It is important to note that the 0 - 6 years age group will be specifically managed at  District  Early  Intervention  Center  ( DEIC ) level  while  for  6 -18  years  age  group,  management  of conditions  will  be  done  through  existing  public  health facilities.  DEIC will act as referral linkages for both the age groups.
• First level of screening is to be done at all delivery points through existing Medical Officers, Staff Nurses and ANMs. After 48 hours till 6 weeks the screening of newborns will be done by ASHA at home as a part of HBNC package. Outreach  screening  will  be  done  by  dedicated  mobile  block  level  teams  for  6  weeks  to  6 years at anganwadis centres and 6 - 18 years children at school.
• Once the child is screened and referred from any of these points of identification, it would be ensured that the necessary treatment/intervention is delivered at zero cost to the family.

Target age group

• The services aim to cover children of 0 -6  years of  age  in  rural  areas  and  urban  slums  in addition  to  children  enrolled  in  classes  I to XII in  Government  and  Government  aided  Schools. It is expected that these services will reach to about 27 crores children in a phased manner. 
• The broad category of age group and estimated beneficiary is as shown below in the table. The children have been grouped in to three categories owing to the fact that different sets of tools would be used and also different set of conditions could be prioritized.

Mechanisms for screening at Community & Facility level

• Child screening under RBSK is at two levels community level and facility level.  While facility  based  new  born  screening  at  public  health  facilities  like  PHCs / CHCs/ DH, will be by existing health manpower like Medical Officers, Staff Nurses & ANMs, the community level screening  will be conducted  by  the  Mobile health teams  at  Anganwadi  Centres  and  Government and Government aided Schools.

Screening at Anganwadi Centre

• All pre-school children below 6 years of age would be screened by Mobile Block Health teams for deficiencies, diseases, developmental delays including disability at the Anganwadi centre at least twice a year. 
• Tool for screening for 0-6 years is supported by pictorial, job aids specifically for developmental   delays. For developmental delays children would be screened using age specific tools specific and those suspected would be referred to DEIC for further management.

Screening at Schools - Government and Government aided

• School children age 6 to 18 years would be screened by Mobile Health teams for deficiencies, diseases, developmental delays including disability, adolescent health at the local schools at least once a year. The too used is questionnaire (preferably translated to local or regional language) and clinical examination.

Composition of mobile health team

• The mobile health team will consist of four members- two Doctors (AYUSH) one male and one female, at least with a bachelor degree from an approved institution, one ANM/Staff Nurse and one Pharmacist with proficiency in computer for data management
