- பிரதான் மந்திரி தேசிய டயாலிசிஸ் திட்டம் ஏழைகளுக்கு இலவச டயாலிசிஸ் சேவைகளை வழங்குவதற்காக தேசிய சுகாதார இயக்கத்தின் (NHM) ஒரு பகுதியாக 2016 இல் வெளியிடப்பட்டது.
- பிரதான் மந்திரி தேசிய டயாலிசிஸ் திட்டத்திற்கான வழிகாட்டுதல்கள் பிபிபி (பொது தனியார் கூட்டாண்மை) முறையில் என்ஹெச்எம் கீழ் டயாலிசிஸ் சேவைகளை வழங்குவதை நோக்கமாகக் கொண்டுள்ளது.
- ஒவ்வொரு ஆண்டும் சுமார் 2.2 லட்சம் சிறுநீரக நோயின் இறுதி நிலை (ESRD) நோயாளிகள் இந்தியாவில் சேர்க்கப்படுவதால், ஒவ்வொரு ஆண்டும் 3.4 கோடி டயாலிசிஸ் தேவைப்படுகின்றது.
- ஏறத்தாழ 4950 டயாலிசிஸ் மையங்களுடன், பெரும்பாலும் இந்தியாவில் தனியார் துறையில், தேவை தற்போதுள்ள உள்கட்டமைப்பில் பாதிக்கும் குறைவாகவே உள்ளது.
- ஒவ்வொரு டயாலிசிஸுக்கும் சுமார் ரூ.2000 கூடுதல் செலவாகும் என்பதால், நோயாளிகளின் மாதச் செலவு ஆண்டுக்கு ரூ.3-4 லட்சம். தவிர, பெரும்பாலான குடும்பங்கள் அடிக்கடி பயணங்களை மேற்கொள்ள வேண்டும்.
- மேலும் நீண்ட தூரங்களுக்கு டயாலிசிஸ் சேவைகளை அணுக அதிக பயணச் செலவுகள் மற்றும் நோயாளி மற்றும் நோயாளியுடன் வரும் குடும்ப உறுப்பினர்களுக்கு ஊதிய இழப்பு ஏற்படுகிறது.
- இது நடைமுறையில் அத்தகைய நோயாளிகளைக் கொண்ட அனைத்து குடும்பங்களுக்கும் நிதி பேரழிவிற்கு வழிவகுக்கிறது. வாழ்க்கைத் தரத்தில் கணிசமான ஆதாயம் மற்றும் நோயாளிகளின் முன்னேற்றம் இல்லாத உயிர்வாழ்வை நீட்டிப்பதன் மூலம், குடும்பங்கள் பாக்கெட்டில் இருந்து அதிக செலவு செய்ய நிதி ரீதியாக நீட்டிக்கப்படுகின்றன.
- இந்த முக்கியமான உயிர்காக்கும் செயல்முறையை வழங்குவதற்கும், நோயாளிகளின் பாக்கெட் செலவினங்களின் காரணமாக வறுமையைக் குறைப்பதற்கும், டயாலிசிஸ் திட்டம் தேவை என்று உணரப்பட்டது.
- தற்போது, NHM இன் கீழ், வறுமைக் கோட்டிற்குக் கீழே உள்ள (BPL) பொருளாதாரக் குழுவில் உள்ள நோயாளிகளுக்கான சேவை நடைமுறைக் கட்டணத்தில் 100% காப்பீடு செய்யப்படுகிறது.
- இருப்பினும், பிபிஎல் அல்லாத நோயாளிகள், பிபிஎல் நோயாளிக்கு அரசாங்கத்தால் வழங்கப்படும் அதே கட்டணத்தில் மாவட்ட மருத்துவமனைகளில் சமூகத்திற்கு நெருக்கமான சேவைகளை அணுகுவதன் பலனைப் பெறுவார்கள்.
- அரசாங்கத்தால் நிதியளிக்கப்பட்ட ராஷ்ட்ரிய ஸ்வஸ்த்ய பீமா யோஜனா (RSBY) போன்ற சுகாதாரத் திட்டங்கள் உள்ளன.
- ஹீமோடையாலிசிஸ் செயல்முறையை உள்ளடக்கிய இந்தியாவில், அதிக செலவு மற்றும் வாழ்நாள் முழுவதும் தேவைப்படும் தொடர்ச்சியான அமர்வுகள் காரணமாக, டயாலிசிஸ் வழங்குவதற்கான மொத்த செலவை போதுமான அளவு ஈடுகட்ட முடியாது என்பது தெளிவாகிறது.
- இருப்பினும், RSBY இன் கீழ் பதிவு செய்யப்பட்ட BPL குடும்பங்களுக்கு, டயாலிசிஸ் சிகிச்சைக்கான செலவு அதன் அதிகபட்ச கவரேஜ் வரை RSBY நிதி மூலம் வழங்கப்படும். தேசிய சுகாதார இயக்கத்தின் கீழ் தேவையான கூடுதல் ஆதாரங்கள் மாநிலத்திற்கு வழங்கப்படும்.
- The Pradhan Mantri National Dialysis Programme was rolled out in 2016 as part of the National Health Mission (NHM) for provision of free dialysis services to the poor. The Guidelines for Pradhan Mantri National Dialysis Programme envisage provision of dialysis services under NHM in PPP (Public Private Partnership) mode.
- Every year about 2.2 Lakh new patients of End Stage Renal Disease (ESRD) get added in India resulting in additional demand for 3.4 Crore dialysis every year. With approximately 4950 dialysis centres, largely in the private sector in India, the demand is less than half met with existing infrastructure.
- Since every Dialysis has an additional expenditure tag of about Rs.2000, it results in a monthly expenditure for patients to the tune of Rs.3-4 Lakhs annually. Besides, most families have to undertake frequent trips, and often over long distances to access dialysis services incurring heavy travel costs and loss of wages for the patient and family members accompanying the patient.
- This therefore leads to financial catastrophe for practically all families with such patients. With substantial gain in quality of life and extension of progression free survival for patients, families continue to stretch financially to make large out of pocket spends.
- It has been felt that both in terms of provision of this important life saving procedure and also for reducing impoverishment on account of out of pocket expenditure for patients, a Dialysis program is required.
- Currently, under NHM 100 % of the service procedure fees for patients from below poverty line (BPL) economic group is covered. However, non BPL patients would have the benefit of accessing the services close to the community at the district hospitals at same rates as paid by the Government for the BPL patient.
- While there exist health schemes such as Rashtriya Swasthya Bima Yojana (RSBY) funded by Govt. of India which cover hemodialysis procedure, it is evident that due to high cost and recurring sessions required over the life time, the total cost for providing dialysis cannot be adequately covered.
- However, for BPL families registered under RSBY, the cost of dialysis care shall be catered through RSBY funding upto its maximum coverage. The additional resources required would be provided to the state under the National Health Mission.
